I understand that the ISD #99 rules require that students ride the bus to and from all school events and a departure from the requirement will release Esko High School from all liability for any adverse results which may occur.

I understand that the student MUST RIDE WITH A PARENT OR A PARENT DESIGNEE OVER THE AGE OF 21.

I agree to release ISD #99 and its employees and officers from all liability with reference to the above stated transportation.

This form must be given to the head coach prior to the departure time.
